How to Apply for a Home Care Package (and Get Government Help)

Applying for government-subsidised home care doesn’t need to be overwhelming — especially when you’ve got the right support behind you. At Harbourside Port Stephens, we’ve helped many local families navigate the aged care system and access the services they need to stay at home longer.

If you or a loved one is ready to explore home care options, this guide will walk you through how to get started with My Aged Care — step by step.

Step 1: Register with My Aged Care

The first thing you’ll need to do is register with My Aged Care, which is the Australian Government’s starting point for aged care services.

You can do this by:

You’ll be asked a few basic questions about your health, any current support you receive, and your ability to manage daily activities. If a family member or carer is helping you apply, that’s completely fine — just have them nearby during the call.

💡 Tip: Have your Medicare card handy — it’ll be needed to set up your My Aged Care record.

Step 2: Book an In-Home Assessment

If it sounds like you may be eligible for a Home Care Package, My Aged Care will organise a visit from an assessor as part of the Single Assessment System (previously known as ACAT).

During this home visit, the assessor will:

  • Talk to you about your daily routine
  • Ask which tasks are becoming difficult
  • Discuss your medical and care needs
  • Consider any safety concerns in your home environment

It’s not a test — it’s simply a conversation to understand what kind of help could improve your day-to-day life.

💬 You can request to have a family member, trusted friend, or a Harbourside team member present to support you during the assessment.

Step 3: Wait for Your Approval Letter

After your assessment, you’ll receive a letter letting you know:

  • Whether you’ve been approved for a Home Care Package
  • What level of care (1 through to 4) you’re eligible for
  • Your place on the national waitlist

Wait times can vary depending on your needs and priority level. In the meantime, you may be offered some interim services if urgent support is required.

Step 4: Choose Your Home Care Provider

Once your package is assigned, it’s time to choose a provider to deliver and manage your care. This is where Harbourside Port Stephens can step in to help.

We’ll work closely with you (and your family) to:

  • Create a personalised care plan
  • Build a budget that maximises your funding
  • Coordinate services such as nursing, social support, cleaning, transport, and more

And because we’re local, you’ll always have someone nearby who understands your needs and can respond quickly as they change.
